QC offers prepaid US Postal Business Reply Labels at $12.00 per box, therefore providing a savings opportunity for you, the doctor, as you may send more than one case per box.
In addition, we can provide you with UPS ground labels for a charge of $13.00 each. Call 1-800-PICK-UPS (1-800-742-5877) or visit www.UPS.com for a pick up. UPS has
a tracking system which allows both you and us to keep up with your cases in transit, and is more efficient and faster than the USPS most of the time.
Here at QC, we appreciate the opportunity to provide you, and your patients, with the quality appliances that you demand and your patients deserve.
This is important as we can only guarantee that the appliances we make will fit on the models you provide. We make every effort to spot distorted models before we begin work on a case, but only you have seen the patient’s actual oral anatomy for comparison.
In order to achieve this goal, we need accurate models. We request that you examine your models for signs of distortion such as knife edges on teeth, compression of teeth, and soft tissue distortion. In order to protect models from damage in transit to us and avoid confusion, we ask that you:
1. Wrap each model individually in foam and write the patient’s name on the bottom of the model
2. Include a prescription form for each case. Make sure that each prescription form has the doctor’s name and license number, address, phone number, patient name, date wanted, and appointment date
Cases are returned to you via UPS at no charge.
Typically cases are in lab three to five working days for all appliances. For Herbst, Twin Blocks, Bionators and all other functional appliances, please allow 5 to 7 working days. Please allow a minimum of two days each way for shipping. To insure cases are returned promptly, please fill in appointment date and date wanted sections on the prescription form.
We make every effort to accommodate rush cases. Please call the lab before sending a rush case. We do not charge extra for rush cases. There will be a service charge , however, for next day, or 2nd day shipping.
DOCTOR’S PREFERENCE SHEETS
Here at QC, we fabricate a wide variety of appliances for an even wider variety of doctors. We fabricate each appliance to the standard design specifications. We are aware, however, that your design specifications may vary from the standard. To accommodate these variances, we will gladly establish a doctor’s preference sheet listing your preferred design for any appliance. We encourage you to call and speak with our technical support team regarding standard designs vs. your personal preferences so that we can custom tailor our fabricating processes to your needs.
APPLIANCE REPLACEMENT POLICY PROGRAM
All appliance materials and workmanship are guaranteed for 90 days from time of fabrication. Original models and appliances must be sent with new models to be eligible for guaranteed coverage. Longer term replacement insurance is available as follows:
- Retainers—$35 per year
- All other appliances 35% of total cost for the first year
- Insurance may be renewed for additional years for the same price.
QC accepts checks, American Express, Master Card, Visa, and Discover. If you prefer, we can keep your credit card information on file for automatic payment.
Invoices are generated and mailed with each case. Statements are printed and mailed out on the last working day of each month.
QC has a program for increasing rewards based on sales volume and on time payment. All accounts are due and payable by the 15th of the month following the purchase. If payment is made by the 15th your discount is as follows:
• Monthly Sales of $500-$999- 2.5% Discount When paid by the 15th
• Monthly Sales of $1000-$1999-5% Discount when paid by the 15th
• Monthly Sales of $2000-$4499-10%Discount when paid by the 15th
• Monthly Sales Over $4500- 15% Discount when paid by the 15th
Net 15 days: The current charges on the monthly statement are due and payable by the 15th of the month. A 1.5% per month service charge will be added to balances over 30 days.
QC guarantees that our appliances will fit the models on which we fabricated them. Therefore, when returning an appliance, we must have the old models to determine where the problem occurred. New models should be sent to refabricate the new appliance.
Cost for repairs or remakes are determined by appliance age, type of fractures, and model distortion (if any). Original models and/or new models should be returned with the original appliance.
